3. Submit cash register receipt dated between 11/16/10 and 2/28/11 with store name and item
circled from Disney’s A Christmas Carol Blu-ray or DVD (photocopies acceptable).
4. Submit cash register receipt(s) dated between 11/1/10 and 2/28/11 with store name and the
items circled from Dearfoams slippers. (photocopies accepted).
5. The hand-printed, completed form.
