The form is on an in store flyer with coupons you can use on some of the eligible items. (My Publix keeps their flyers at the customer service desk. You have to ask for them. But it’s no hassle. I just say, “Can I get the coupon flyer?” And they’ll hand me a Yellow Advantage Flyer with all the other flyers and coupon sheets tucked inside of it – they take the time to make them into little packets!)
