You will need this form, the UPC off your product and the original receipt dated between 11/1/11 and 12/31/11.
Requests must be postmarked by 1/31/12.
Limit one per person or household. $5.00 will be in the form of a prepaid card.
I would suggest printing the form now if you think you might do the rebate. Sometimes they will pull the form off the internet before the rebate offer is over.
