To claim and receive your rebate by mail:
- Purchase ONE (1)
Spot Shot ® trigger or aerosol can (excluding 4 oz. size) and ONE (1) Cats & Dogs: The Revenge of Kitty Galore DVD or Blu-RayTM from any retailer. - Complete and print this original rebate form – no reproductions accepted.
- Include your original sales receipts. Sales receipts must clearly show store names, store addresses, products (and their prices) purchased and purchase dates. (Make a photocopy of receipts to keep for your records.)
- Mail all items together with appropriate postage postmarked by 7/8/11 (or 8/1/11 for NC residents) to:
