A Spot Shot Rebate by Mail is available with the purchase of a Cats & Dogs: The Revenge of Kitty Galore DVD or Blu-ray_TM and any Spot Shot product (excluding 4oz). Spot Shot product and the Cats & Dogs: The Revenge of Kitty Galore DVD or Blu-ray must be purchased from an authorized retailer between 11/1/10 and 6/30/11.
To claim and receive your rebate by mail:

1. Purchase ONE (1) Spot Shot_® trigger or aerosol can (excluding 4 oz. size) and ONE (1) Cats & Dogs: The Revenge of Kitty Galore DVD or Blu-Ray_TM from any retailer.
2. Complete and print this original rebate form – no reproductions accepted.
3. Include your original sales receipts. Sales receipts must clearly show store names, store addresses, products (and their prices) purchased and purchase dates. (Make a photocopy of receipts to keep for your records.)
4. Mail all items together with appropriate postage postmarked by 7/8/11 (or 8/1/11 for NC residents) to:
