This offer presents itself as a "Try Me Free Offer" (which is what it says at the top of the offer) and try me free offers are just that – an opportunity to try something for free.
By including the question on the form "Tell us what you didn't like" and making it required is something you would find on a Money-Back Guarantee. (Totally different than a try me free offer). I usually do not post "money-back guarantees" because I don't consider them rebates. Because "money back guarantees" are for you to try the item risk free and then get your money back if it doesn't meet your expectations. It's an ethical thing – a gray area in rebating to request your money back on a perfectly fine product just because the offer is there.
Try Me Free Offers should not have such a question as is on this one.
If you don't want to submit the rebate on the grounds of ethics (you liked the product) then don't.
But in this situation since the form itself is a bit ambiguous and not clear as to if this is a TMF or MBG, you can just put a generic response such as "The product didn't meet my expectations" or "I still prefer my usual brand over this one" etc.
