To be reimbursed for the cost of ACTIVIA® yogurt, purchased between 7/1/10 – 9/30/10, follow these directions:
Requests must be postmarked by 10/15/10 and received no later than 10/31/10.
To receive a check for up to $6.00 (actual refund check will be your ACTIVIA® purchase amount up to a maximum refund of $6.00, not including sales tax or the amount of any coupon or discount received at the time of purchase) please complete this original redemption form and mail it with the original store-identified cash register receipt(s) dated between 7/1/10 – 9/30/10 with the purchase price circled.
Valid in USA only. Limit to one (1) submission per family, household or address for a given game period. If you have also participated in the ACTIVIA® 14-Day Challenge (which runs from 4/15/10 – 8/31/10) you are still eligible to participate in the ACTIVIA® Taste Test, as they are two separate promotions. Specific store receipt may be used for only (1) participation per household per each specific Dannon® promotion. Only one request per stamped envelope. Offer expires 9/30/10.
