There is a new mail in rebate form for Snapple Iced Tea 6pk. They are peelies found directly on the package but if you can’t find the peelie form you can also mail and index card with the following information:
Name
Address
Phone and/or Email
Flavor Purchase
Price Paid
Store Name
Plus you must send the original receipt dated between 5/1/10 – 7/7/10 and the original proof of purchase. The rebate is good for one 6pk of Snapple up to 6.99.

You can mail in one per person in the household, just use different names on the index cards and you can mail them in the same envelope! (This is very unusual!!)
